Sportsmanship and participation form to be signed by each coach and
mentor. Parent or Guardian signature required for mentors under the age
of 18.
Background Check
Required for coaches and mentors over the age of 18 that will work or
have contact with the players. It is sent to CAHA and they handle the
processing.
